Nov 13, 2001Nelson has been approved for the Federal leave donor program. Anyone that would like to make a leave donation can do so by completing form OPM 630B (see attached). Line number 10 has already been completed. Part A needs to be completed by the federal employee and Part B by the employee's Agency.
